Form Completion Guidance
  • This form must be returned to the MIS team as soon as possible once the change is known about.
  • This form can be used for changes such as address, name, etc.
  • This form can be used to capture missing or correct information on a Learning Agreement.
  • It can also be used for suspension of a learner – if this is happening for a specific period of time please include start and end dates for suspension period.
  • If the time period for suspension is none specific you will need to use two forms. One for state date of suspension and one for completion of suspension and change of planned end date.
